Cervical Smears

Please book these with a practice nurse.

Chronic Disease Management Clinics

We offer a range of clinics such as those for asthma, diabetes, coronary heart disease (CHD), stroke and chronic obstructive pulmonary disease (COPD) to monitor and support patients.

They are run by the practice nurse, with the support of the doctors. An appointment is necessary but these can be booked in advance. All patients with the above conditions should ensure that they book an annual review with the nurse.

If you have been asked to complete an asthma review,  please complete our form.

If you have been asked to complete a COPD assessment, please complete our form.
